You will receive a Super Bill (and invoices if requested, to reflect session costs) near the end of each month for presentation to your insurance company for Out of Network Provider reimbursement.
******NOTE: Starting January 2023 I will be accepting
fee-for-service only. You can check with your insurance provider for
out-of-network coverage.
Insurance
Most health care insurance offers coverage for mental health. Prior to working with me, I recommend that you contact your insurance carrier to obtain information about your mental health benefits, your annual deductible, and your co-pay or co-insurance payment. Your health insurance coverage is an agreement between you and your insurance company.
Insurance usually pays for individual and couples therapy, and often for tele-heath (phone) sessions as well. I provide in office, Telehealth or phone sessions.
OUT OF NETWORK
As a licensed clinical therapist, I am an "out-of-network" provider on most other insurance plans. This means that you will most likely receive reimbursement for part of my services by your private insurance company. Reimbursement to you is often about 50% to 85% of my fee. Medicare and State insurance will not reimburse you.
- If I am not in-network for you, I do not bill your insurance company.
- I ask you to pay my full fee directly on the day of service via your stored credit card, and I will give you the paperwork (superbill) at the end of each month needed for you to seek reimbursement from your insurance company
- If I am not in-network for your insurance company, I suggest you ask your insurance company what you will be reimbursed by them. You can ask:
- What percentage will I be reimbursed for psychological services from an "out-of network" provider who is a Licensed Therapist in Washington State?
- The CPT code I use for 45-50 minute therapy sessions is 90834 and for 25 minutes of therapy, it’s 90832.
- Cost is $150 for intake, $125 for 50 minute session and $100 for 25 minute session or emergent session.
PAYMENT
Payment for services is an important part of any professional relationship. I ask that you provide credit card information, through Simple Practice, so that I can charge your outstanding balance at each session.
